Autophagy and fasting: their role in healthy aging

What is autophagy and how does fasting affect it? An evidence-based look at autophagy's role in healthy aging and the practical limits of fasting.

In brief:

  • Autophagy is the cells’ natural self-cleaning process, which breaks down damaged components and recycles their building material.
  • Food scarcity and fasting are strong triggers of autophagy, which is why fasting is linked to cellular health in aging research.
  • Human studies on autophagy and lifespan are still limited — most of the strong evidence comes from animal experiments.
  • In older age, fasting requires keeping an eye on protein intake and muscle mass to avoid sarcopenia.

Autophagy is one of the main mechanisms cells use to keep themselves in order, which is exactly why it has become a hot topic in aging science. Fasting is one of the best-studied ways to boost this process. In this overview, we look at what science actually says about the link between autophagy and healthy aging, and where the practical limits lie.

What is autophagy and why does it matter in aging?

Autophagy (Greek for “self-eating”) is the cell’s internal recycling system: specialized membranes enclose damaged proteins, aged mitochondria and other cellular “garbage,” directing them to be broken down and reused (Mizushima & Komatsu, 2011). Yoshinori Ohsumi received the Nobel Prize in 2016 for describing this mechanism.

With age, autophagy tends to become less efficient, so damaged components build up in cells. This build-up is one theory of the cellular-level causes of aging, and restoring or boosting autophagy is being studied as a possible route to supporting healthy aging (Madeo et al., 2019). The World Health Organization stresses that healthy aging means maintaining functional ability, not merely the absence of disease (WHO Healthy Ageing).

How does fasting trigger autophagy?

When nutrients are scarce, the cell senses an energy shortage and switches into saving mode: mTOR, the sensor of nutrient abundance, goes quiet and autophagy activates, so the cell can obtain fresh energy and building blocks from the broken-down material. A longer break from eating triggers the so-called metabolic switch, in which the body shifts from using glucose to using fatty acids and ketone bodies (de Cabo & Mattson, 2019). This metabolic shift is one reason intermittent fasting is studied so much in the context of autophagy.

If you are curious how fasting fits with training, take a closer look at the article Fasting and the gym, and for the basics of fasting, Fasting will help. It is important to understand that the exact window in which human autophagy peaks has not been conclusively determined in human studies — much of the precise timing data comes from animal experiments.

Does autophagy extend lifespan?

In animal models, boosting autophagy — whether genetically, through calorie restriction or through fasting — has repeatedly extended lifespan and improved health markers (Madeo et al., 2019). Reviews of intermittent fasting link it to better metabolic markers, lower inflammation and better brain health (Mattson et al., 2018).

In human studies, however, the picture is more cautious. Most high-quality data concern short- and medium-term metabolic changes, not lifespan extension directly (de Cabo & Mattson, 2019). So it is fair to say that autophagy has a strong theoretical and animal-study basis as a supporter of healthy aging, but no firm promises about human lifespan can be made. A balanced approach to mental and physical health remains the foundation — you can read more about it in How to keep body and mind healthy.

What are the risks of fasting in older age?

In older adults, the biggest danger is the loss of muscle mass and strength, known as sarcopenia. The European consensus EWGSOP2 defines sarcopenia primarily through declining muscle strength and stresses the importance of early detection (Cruz-Jentoft et al., 2019). Fasting that is too aggressive or too long, without sufficient protein intake, may speed up this process.

It is therefore sensible to keep fasting windows moderate, ensure enough protein during eating periods, and add strength training to protect muscle mass. You should also keep an eye on nutrient coverage — you can read more about the role of vitamins and minerals in Micronutrients and their role in nutrition. With chronic illness, pregnancy, diabetes or eating disorders, fasting should be started only under a doctor’s supervision.

Frequently asked questions

What is autophagy in simple terms?

Autophagy is the cells’ self-cleaning process, in which damaged parts are broken down and their building material is reused.

How long do you have to fast to trigger autophagy?

The exact time window based on human studies has not been conclusively established; a longer break from eating triggers a metabolic switch, but much of the precise timing data comes from animal experiments.

Does fasting extend lifespan?

Results in animal models are promising, but there is no firm evidence in humans that fasting extends lifespan; the benefits have mainly been seen in metabolic markers.

Is fasting suitable for older adults?

With caution — the main risk is the loss of muscle mass, known as sarcopenia, so you need to ensure enough protein and strength training and consult a doctor if necessary.
